Release of Medical Records
To request a complete copy of your medical record or radiology reports and images, or to send a copy to a third party, please download and complete the Authorization for Release of Health Information form. You may return the completed form to the Medical Records Department by fax at 845-348-8433 or by email at ROI@montefiorenyack.org. We can provide copies of your medical records via secure email and/or we can mail a CD to you.

Birth Certificate Inquiries
To request a birth certificate for an infant 3 months of age or younger, call 845-348-2885. For all other birth certificate requests, contact the Village of Nyack:
