DESCRIPTION

The Oncology Social Worker functions as a member of the interdisciplinary treatment team.  The Social Worker will provide patients and family members with the education and advice needed to cope with issues which arise from various emotional, physical problems and financial matters related to their diagnosis. Coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments including survivorship care plan. Collaborates in the development and implementation of performance improvement initiatives and quality indicators.

 

QUALIFICATIONS
1. Education: A Masters Degree in Social Work ( MSW) , from an accredited institution and current licensure (LMSW, LCSW) or certification required by state for the specific field.
2. Experience: Two to three years of oncology social work experience preferred in a hospital or community- based setting. Experience with providing individual, couple and family support.
3. An understanding of the cancer experience or for those diagnosed, their families and the bereaved preferred.
4. Previous Healthcare Case Management Experience
5. Training/Certification: Oncology Social Work certification, (OSW) within 3 years of hire.
6. Demonstrate sensitivity and commitment to mission, core values and program.
7. Must have flexibility in hours and scheduled works.
