If you or a loved one need care to recover from a hospital stay, manage a chronic medical condition, or to assist with daily living activities, Montefiore Nyack Hospital Home Care offers a wide range of services to help you stay at home safely. We've been delivering the most compassionate and highest quality healthcare at home to adults aged 18 and older for more than 40 years.

Our experienced team of providers offers the help you need for a range of conditions, including skilled nursing care, rehab therapy, wound and ostomy services, and physical and occupational therapy.

Home Care Services

Montefiore Nyack Hospital Home Care works closely with your physician to coordinate and provide services based on your unique needs. We offer the following services:

• Skilled nursing care
• Physical therapy
• Occupational therapy
• Speech therapy
• Medical social services
• Wound and ostomy care
• 24-Hour on-call nurses
• Home health aide

Your Home Care team partners with your physician to deliver an individualized treatment plan. Home Care professionals may include:

• Registered nurse
• Physical therapist
• Speech therapist
• Occupational therapist
• Social worker
• Home health aide

Insurance

Montefiore Nyack Hospital Home Care is reimbursed by Medicare, Medicaid and most insurers.

Hours

Monday—Friday: 8 a.m. to 5 p.m.

Saturday—Sunday: 8 a.m. to 4 p.m.

Referrals

We welcome referrals from physicians, family members, hospital case managers, nursing homes, rehabilitation facilities, assisted living facilities, community organizations and patients themselves.
