DESCRIPTION
Provide clinical Speech Language Pathology services to Home Care patients. Provides speech therapy services: evaluation, treatment, teaching, supervision and consultation to patients in their homes, in accordance with state/federal regulations, professional and Joint Commission standards. Develops, reviews, implements and revises the patient Plan of Care in collaboration with the patient/family, and under the direction of the physician; Coordinates and maintains effective communication in regard to patient services, with community agencies and other involved disciplines: physical therapy, occupational therapy, nursing, social work, home health aide.
QUALIFICATIONS
QUALIFICATIONS: One year work experience as Speech Therapist preferred. Experience preferred with adults and children with neurological and developmental disorders. New York State Licensure and current registration in Speech Language Pathology.
