Participate in the development, implementation and evaluation of training, orientation and, in-service programs and competency assessment for all Home Care staff. Provide support of all clinical staff in the proper use of the Horizon Home Care clinical application. Conduct clinical record reviews and develops and implements educational programs in areas identified as requiring additional education to promote superior patient outcomes. Collaborate across teams to plan, implement and evaluate the effectiveness of the clinical documentation system and works with the IT staff to modify the system to meet changes in documentation requirements. Participate in the agencies QAPI Program.
