The Medication Documentation Specialist will be part of the Pharmacy Clinical Specialist Service and will conduct patient, family or caregiver interviews to obtain the most accurate list of home medications for all patients being admitted to the hospital. The outcome of this interview is a timely, complete and accurate medication list that is ready and available for physician review. When collecting patient’s home medication lists, the Specialist may utilize other data sources to further investigate and complete an accurate medication list. The medication list compiled by the Specialist will be entered into the inpatient electronic medical record and communicated to the admitting physician and nurse. Any discrepancies that need further clarification may be escalated to the covering unit clinical pharmacists.

4. Other: Must have considerable knowledge of medication names (both trade and generic), their uses, doses, and routes of administration. Must be able to identify drugs based on markings. Must have excellent patient interview skills as well as written and verbal communication with both patients and other healthcare professionals. Good computer and customer service skills are essential.
