The Ortho Case Manager position is within the Case Management department of the hospital. As an RN Case Manager you will manage the Ortho cases utilizing the nursing process to provide optimal quality and continuity of nursing care from the pre-operative setting through a sixty-day episode of care. The Ortho RN Case Manager will assist the patient throughout the transition of care to ensure their well-being and achievement of the desired outcomes. Will work collaboratively as part of a multi-disciplinary team to provide patient and family education, communication with health care providers, and coordination and facilitation of services within an episode of care. Provides superior customer service by modeling the We Care Standards.
